Downloadable Forms

Room Reservations & Special Accommodations Form
Use this form to reserve conference rooms, computer labs, or classrooms in the Main Library for an extended period (i.e. more than 2 hours).
Poster Printing Charges Form
The Health Sciences Library poster printing service is currently available for New York Medical College Library users via electronic submission only. For more detail regarding our poster printing procedures and service fees, check out our guide here.
Thesis Permission Form
Grants non-exclusive rights to the Library allowing it to archive and make a thesis/dissertation accessible online in the College’s institutional repository, Touro Scholar. The form can be filled out, signed, scanned as a PDF, and uploaded as an Additional File into Touro Scholar.
Special Permission Form
If you are not directly affiliated with NYMC, use this form to request access to library electronic resources. The form must be accompanied by formal letter from the Department chair stating reason for library access. Special Permission is granted by the Library Director on case by case basis for six months.

Online Forms

Touro Scholar Submission Form
You can use this form to submit your scholarly works for inclusion in Touro Scholar. Please provide the type of submission (e.g. book, journal article, etc.) and the submission's citation. You can also upload a full-text file containing the version of your submission that your publisher has approved for online posting.
Touro Scholar New Collection Request Form
You can use this form to submit a request for us to create a new collection in Touro Scholar. Please provide as much information as possible about the collection (e.g. name, purpose, update schedule, go live date, etc.).

Literature Search Form
Faculty and residents are welcome to submit search requests to be completed by a librarian.
Systematic/Scoping Review Collaboration Form
Complete this form if you are interested in partnering with a librarian co-author on your comprehensive research project. If you are looking for a basic literature search service, please complete the Literature Search form.
Class & Instruction Request Form
Librarian-led instruction sessions boost competency in finding information and evaluating, synthesizing and applying information to produce a quality end product. See our HSL Information Management Competencies. If you have a more immediate need, you can schedule a one-on-one consultation with a librarian.
Recommendations Form
The Health Sciences Library welcomes recommendations of book, journal or media titles that support the educational programs of New York Medical College.
